Chameleon’s Lament, also known as the “Changeling Disease”, is a rare autoimmune condition that affects changelings exclusively.
Symptoms and Progression
Chameleon’s Lament manifests when a changeling maintains a single appearance for an extended period without engaging in regular transformations. The exact duration of this “stagnation period” may vary from individual to individual, but it is generally believed to range from several months to several years. As the changeling’s body adapts to a fixed form, their immune system mistakenly identifies the body’s own cells as foreign entities, triggering a chronic autoimmune response.
The initial symptoms are subtle and may be mistaken for general fatigue or weakness. Affected changelings may experience mild discomfort, joint pain, and reduced energy levels. However, as the condition progresses, more severe symptoms emerge, such as organ dysfunction, internal inflammation, and cellular alteration.
Cellular Alteration and Organ Dysfunction
Chameleon’s Lament leads to a unique phenomenon in which the changeling’s internal organs and cells undergo progressive morphological changes. The immune system, in its misguided attempt to protect the body, begins attacking healthy tissue, causing abnormal cell growth and tissue degeneration.
Over time, this morphing process can affect various organs, leading to complications in the cardiovascular, respiratory, digestive, and nervous systems. The condition may even impair the changeling’s ability to shapeshift altogether, further worsening their state.
Mechanics
Chameleon’s Lament is a progressive autoimmune condition affecting only changelings. It advances in stages, with each stage introducing increasingly severe mechanical and narrative consequences.
The disease is not magical in origin and cannot be cured by effects that remove curses or ordinary diseases unless explicitly stated otherwise.
Progression
Once contracted, the disease progresses in Stages (I through V).
- Check Frequency: The infected creature must make a Constitution saving throw once every sleep.
- DC: 10 + current Stage level * 2.
- Failure: Accumulates, after 3 failures the disease advances to the next stage. Nat 1 is two failures.
- Success: Accumulates, after 3 successes the disease all failures and successes accumulated are removed. Nat 20 is two successes.
Treatment & Remission
This condition cannot be cured by Lesser Restoration. Greater Restoration reduces the disease by one Stage but cannot remove it entirely if the creature remains in the same form.
- Therapy: Spending 7 consecutive days performing a daily Meaningful Shapechange (held for at least 1 hour) reduces the disease by one Stage. A change at any stage besides stage 0 requires a constitution save.
- Remission: Regressing to Stage 0 cures the active symptoms, though the creature remains susceptible to recurrence if they return to stasis.
Definition: Meaningful Shapechange
To qualify as "meaningful" for the purpose of treating SIAS, a new form must differ from the previous one in at least two of the following categories:

Stages of Infection
Stage I: Cellular Fatigue
The body begins to break down, slowly but surely. Exhaustion sets in, along with joint pain and temporary weakness bouts.
- Effect: The Changeling receives 1 CL point, which functions like Exhaustion but does not count towards Exhaustion cap and is not removed by a short rest.

Stage II: Morphic Instability
Internal organs begin to subtly reshape themselves, no longer fully compatible with any single form.
- Effect: The Changeling receives 1 CL point.
- Flavor: More pronounced weakness, short coughing fits.
Stage III: Systemic Rejection
The immune system actively attacks the body’s own tissues.
- Effect: The Changeling’ Constitution becomes 10, their CL increases by 1, and upon failure to assume a Meaningful Shapechange they recieve 3d10 points of damage (poison)
- Flavor: Respiration issues, joints locking, tiredness, stomach pain on digestion, nausea.
Stage IV: Organ Morphosis
The ability to hold a form fractures. The body becomes hostile to its own anatomy.
- Effect: The Changeling’ Constitution becomes 6, and each time they gain HP, roll on the Organ Failure Table.
- Flavor: Organs twitch uncontrollably and microchange, bloody cough, hard to eat, skin flakes aggressively, joints are swollen.

Stage V: Terminal Identity Collapse
Total biological collapse. The body no longer recognizes itself.
- Effect: Maximum Hit Points are halved.
- Dissolution: At dawn each day, make a Constitution save. On a failure, gain 1 level of Exhaustion. This exhaustion cannot be removed by rest or magic while at Stage V.
- Death: At 10 levels of exhaustion, organs fail catastrophically and the creature dies.
Lore & Variants
The Feywild Resonance
The Feywild’s mutable nature soothes the condition.
- While in the Feywild, Progression checks are made with Advantage.
- Each day spent in the Feywild counts as 2 days for the purpose of therapeutic regression.
Treatment and Management
Chameleon’s Lament requires a careful and precise treatment approach. The primary goal is to halt the autoimmune response and reverse cellular alteration while encouraging the changeling to embrace their innate shapeshifting abilities.
Immunosuppressive Drug Therapy
In the early stages, immunosuppressive medications are used to control the aggressive response of the immune system. This helps slow cellular morphing and reduces further damage to organs and tissues.
Shapeshifting Therapy
To effectively combat Chameleon’s Lament, those diagnosed are encouraged to embrace their natural shapeshifting abilities. Regular transformation, ideally on a daily basis, helps maintain cellular stability and prevents the immune system from perceiving fixed forms as harmful.
Restorative Medicine
In combination with shapeshifting therapy, restorative medications are administered to reverse the alteration process or stabilize the damage already incurred, repairing damaged tissues and organs.
Counseling and Support
Coping with Chameleon’s Lament can be emotionally taxing. Relying on others is recommended.
In most cases, changelings who do not shapeshift frequently are those who associate a specific form with a sense of belonging to their close social circle. By the nature of their existence in the world, they often do not know many other changelings; thus, a diagnosis of Chameleon’s Lament can constitute a double blow: both a sense of isolation following the diagnosis of a condition few have ever heard of, and isolation resulting from the medical necessity to increase shapeshifting.
Due to the emotional difficulty involved, some of those diagnosed refuse treatment, while others choose to sever ties with their former lives and set out in search of a lifestyle that allows for frequent changes of form (especially if they encountered misunderstanding from their loved ones). A few have even ventured in search of the Feywild, hoping to find and integrate into changeling society, and once again feel a sense of belonging.
Prognosis
With timely and consistent treatment, changelings with Chameleon’s Lament can lead fulfilling lives. Those who adhere to shapeshifting therapy and proper medical management can effectively halt the progression of the condition and prevent severe complications.
However, if left untreated, Chameleon’s Lament may lead to irreversible organ damage and may ultimately prove fatal. Therefore, early diagnosis and intervention are critical to a positive outcome.
